Johnnie-Mac Fazel, age 66, of Jeffersonville formerly of Charlestown, IN passed away Thursday, August 30, 2012 in Norton Hosparus in Louisville, KY.
He was born August 7, 1946 in Richland Center, Wisconsin the son of Hugo and Daisy Barber Fazel.
Survivors include his daughters, Charity Faith Fazel and Rose Fazel.
Cremation was chosen and services will be held at a later date. Grayson Funeral Home, Charlestown is in charge of arrangements.
