Jerry David McIntosh, 52, of Jeffersonville, IN, passed away on Monday, February 13, 2017 at Kindred at Jewish Hospital in Louisville, KY.
He was born on December 12, 1964 to William Douglas Rogers and Lois Marie Estes in Dayton, OH, then later adopted by Manuel Fritz McIntosh and Debbie McIntosh. He was a former cook. He is preceded in death by his adopted father, Manuel McIntosh; niece, Crystal Kay Green.
Survivors include his daughter, Natasha McIntosh; son, Joseph Zollman (Brandi) of Charlestown, IN; mother, Lois Raisor of Frankfort, KY; adopted mother & aunt, Debbie McIntosh of Scottsburg, IN; sisters, Nettie Harbin (Tom) of Henryville, IN, Deborah Green (Merl) of Underwood, IN, Jeanette Hawkins (Stephen) of Underwood, IN, and Rebecca Ledford (Kevin) of Louisville, KY; several nieces and nephews.
Funeral service will be held on Monday, February 20, 2017 at 1pm at the Grayson Funeral Home in Charlestown. Visitation will begin after 11am on Monday.
