Mrs. Cook was a native of Scottsburg, IN, a housewife, and a medical assistant for Drs. Jones and Voskuhl in Charlestown. She is survived by her husband Donald L. Cook, Sr., a daughter Marcia Hampton, two sons; Gerald Sutton and Donald L. Cook, Jr. She is also survived by three grandchildren; Amy Sad, Jill Ogden, and Lauren Hampton, and four great-grandchildren; Kendall and Tori Ogden, and Isabella and Connor Sad.
